To receive a payment, please do the following:
1 Verify that your Flushmate System is part of the Flushmate III voluntary recall. If you have questions about the recall, please visit www.flushmate.com/recall.
2 If you have incurred out-of-pocket installation expenses for having installed a Flushmate repair kit, a replacement Flushmate pressure vessel, and/or a replacement toilet, complete and submit the Non-Property Damage Claim Form by September 24, 2020.
3 If you have sustained direct property damage caused by a Flushmate System that Leaked or Burst, complete and submit the Property Damage Claim Form by September 24, 2020.
